In Unit 2, we learned about the role of primary care facilities. In this Unit, we have learned more about hospital structures and services. This assignment will demonstrate your knowledge on how each of these delivery models serves patient needs.

Picture yourself as an administrator for a small community hospital. You need to create a flowchart to be used for patient education purposes. The objective of this flowchart is for prospective patients to understand where to seek care based on their medical needs. Include the following in your flowchart:

• Hospital services
• At least three (3) clinical care departments within a hospital
• Health conditions treated within these departments
• Primary care services
• At least three (3) services offered by primary care practices
• Health conditions treated within primary care practices
