NOTE: DO NOT simply submit the template. This is JUST AN EXAMPLE for you to follow. You may NOT choose the same topic and scenario as the template content. Be sure to carefully review the template and grading rubric prior to starting. This assignment provides you with a practice-based opportunity to think critically about real-world costs, resource allocation, and funding limitations involved in public health planning. Scenario: Your team has received a $50,000 grant to implement a 6-month diabetes prevention intervention in a local community health center. The goal is to reduce diabetes risk factors among 50 participants by providing weekly education, physical activity support, and healthy food access.

- Choose your own topic and proposed intervention. Describe your scenario in at least ONE FULL PARAGRAPH with at least 5 COMPLETE SENTENCES.
- Build your budget. Estimate costs for each category listed on the template. NOTE: You may have to adjust these, depending on your scenario, but you must include ALL CELL CONTENT as listed on the template.
- Stay within a $50,000 budget.
- Justify each expense in 12 sentences.
- Answer the 5 questions. Each question should be answered in a minimum of 3 complete sentences.
- Check to make sure your submission is in alignment with the grading rubric criteria to avoid losing points.
- Submit the assignment as a MSWord document only, by the stated due date below.
