Assignment Instructions
One of the goals for your Personal Health Portfolio is to identify and plan for a positive behavior change. At this mid-point in the course, you will identify your targeted behavior change. This is called your Behavior Change Contract.
This activity gives you an opportunity to identify change for one part of your life that has either a) influenced one of your dimensions of health, or, b) been influenced by one of the dimensions of your functioning – mental/emotional, physical, social, spiritual, intellectual – or whichever dimensions you accept in your definition of health. As you can see – the relationship of behavior and health can work either way. Your behavior will influence your health and your health may also influence your behavior and academic success.
Your challenge for this course it to determine an area of your day-to-day life that you would like to change to improve your health and general life satisfaction. The assessments you have been doing in class and those that are still to be done should provide you with some areas to consider. You may be involved with improving relationships, work environment, living environment, sleep habits, smoking cessation, drinking alcohol more responsibly or eating habits – whatever it is – you should complete the attached behavior contract and plan and submit it.
You must be able to support your behavior change strategies with credible resources and include a complete source citation with your S.M.A.R.T. goal.
BEHAVIOR CHANGE PREPARATION
- Download and read the attached “Behavior Change Guidelines” .
- Before you submit your SMART goal-review this information. .
- Fill out the SMART goal worksheet (pages 1 and 2)
- Log in to and type “Write a SMART goal for (paste what you wrote for your SMART goal)
- Compare your SMART goal to how reworded your goal.
- Which SMART goal is more clear? If you chose to post the ChatGPT version remember to reference it.
