History and physical note pediatrics

This is not an essay. The purpose of this assignment is to create a history and physical on a pediatric patient within the age range listed at the top of the attached template. Simply follow the template and fill in the blanks. The patient must have at least 1 secondary diagnosis (the first diagnosis is already listed). Review of systems is subjective info that would come from the mother. The physical exam portion must list at least 3 things per system. Make sure the physical exam matches up with the appropriate signs/symptoms for the listed diagnosis. 3 sources must be listed but do not need to be cited throughout the paper due to this not being a write up assignment. There is no word count required, nor page length requirement, as long as all the points and systems listed in template are addressed.
