Nurses’ Notes

Today:

Client in the emergency department reports feeling feel thirsty all the time but has not been drinking water because of nausea. Client reports frequent urination throughout the night. Client also reports continued weakness of arms and legs, and they have recently begun experiencing overall body weakness.

Physical Exam:

Mental status: Oriented to person, place, and time. Responses to questions are slow.

Respiratory: Lung fields are clear to auscultation.

Skin: Warm, dry, and intact. Mucous membranes pale and dry.

Cardiovascular: S1, S2 present, no murmur. Capillary refill greater than 4 seconds.

Abdomen: Soft, nontender. Bowel sounds hypoactive in all four quadrants.

Vital Signs

Today:

Blood pressure 120/70 mm Hg
