In this assignment, you will introduce the topic you identified in the windshield or walking survey. You will create the first several slides of your PowerPoint (ppt) presentation about your CIP project. You will build and improve each slide as you go until you have a complete ppt presentation. You are to imagine presenting this to a governing board or city council for approval or to a group to provide education. You want this to keep the audience’s interest. Use bullet points that get your message across and not complete sentences. Each student should include their name on the slide they worked on. Several students may work on the same slide and should include their names if that is the case. Each group will designate a member who will submit a copy of the project in the designated dropbox by the published deadline. Use APA in text citations and the last slides will be the Reference list. Take some photographs that tell the story of the community health topic you’ve identified and select 1-3 of them. Use the PHOTO acronym to guide you in the analysis of the photo(s) and how they represent your topic. The topic could be something that is a threat to health or safety or is something good that is happening and could be extended. P=describe your PHOTO. H=what is HAPPENING in your picture? O=what are the OPPORTUNITIES for us to improve community health? T=what does this picture TELL us about your community health issue? O=what OTHER information do you need to share about the issue and opportunities? From this analysis you will produce 6 slides to start your ppt presentation. These are the first slides of your presentation. You will be editing and adding to this growing ppt each week. Be sure to give a clear, succinct explanation of the issue, including the risk or benefit involved. Refer to your observations made during your walking/windshield survey. You want to help us see the community and understand the issue you are working on. Give in text citations for any facts and/or data that you include in this slide. Slide 5 (1-2 slides max.): Community Assessment Windshield/Walking Survey Outline the results of your windshield/waking survey. What did you see, feel, smell, or touch? Include information such as:History: age of houses, building (new, old)? Mode of transportation (walking, school buses, daycare vans, private vehicles)? Physical activity (parks, sports complexes, gyms)? Legal (courts, jails, prisons)? Education how many schools? Demographics: age groups, racial and ethnic groups, economics, homelessness, and family types. Population Diversity: What populations are represented? Who do you think might be vulnerable populations? Values & Beliefs: Are there churches, synagogues, or mosques? Is private or public property cared for (condition of structures, streets and highways, sidewalks, parks, etc.)? Are there libraries? Is there evidence of commerce? Slide 6 (1-2 slides max.): Safety/Health Issues Facts Include more descriptive data/facts about your identified health/safety issue. Use resources that are less than 5 years old. Give in text citations for any facts and/or data that you include in this slide. Slide 7 (unlimited): References Include references using the 7th edition of the APA Manual. Web sources: Community Toolbox Photovoice: Community Toolbox Windshield/Walking Survey: STEP 2: You have identified and defined a community/public health issue/topic and documented it in your first ppt slides. Provide a more extensive assessment of your community and selected issue/topic, and outline a strategy to improve, if it is a risk or extend, if it is a benefit. You will use relevant local, state, and/or national information and statistics to substantiate your findings. Remember to correctly cite your sources in APA format and add your references to your reference list slides. With these assessment findings you will design a plan to address the issue/topic and document this in your next ppt slides. What actions are needed to address this issue? For this plan, you will add 4 slides to your presentation. Make sure to edit your previous slides according to grading rubrics and my comments/suggestions. Follow these steps of the nursing process: Slide 7 (1-2 slides max): Community Demographics In an outline format, provide more demographic information about your community. Be sure and cite source of statistics/data on slide and add citation using APA format. Include information such as:Population % Race Ethnicity Gender Level of education Income level Poverty level Housing Major form of employment Health concerns Availability of health care Incidence of specific diseases Slide 8 (1 slide max.): Goals Overarching goal:For example: There will be no automobile deaths due to texting/phone use in Little Rock in 2020. SMART goal:Provide a more realistic goal for your project. For example: 60% of overweight members of my church exercise group will walk in the gym 3 times a week the next 2 months. Slide 9 (1 slide max.): Objectives Write a minimum of 2 incremental SMART objectives to help you reach your goal. For example: Increase vegetable consumption by people aged 2 years and older to 0.25% by October 31, 2023. Goals and objectives must be Specific, Measurable, Attainable, Relevant and Timed. Slide 10 (1-2 slides max.): Interventions List proposed interventions for each objective. Include a minimum of 2 interventions for each objective (Refer to the ppt template provided). Indicate who is responsible for each intervention.Add a timeline specifying when each intervention will be complete. Support each intervention with a scholarly reference that is less than 5 years old. Slide 11 (unlimited): References Include additional references using the 7th edition of the APA Manual. Remember: You are building your PowerPoint as you go. Review the comments and suggestions made on CIP Step1 by your instructor and make the necessary changes before submitting CIP Step 2. Submit all your slides, starting with your title slide and ending with your updated reference list slide, with this and every CIP project assignment.
