Patient Focus for the Presentation: Each student may choose their own focus for this presentation. However, it is suggested that you consider focusing on one from the prior patient case scenarios provided in the Course Documents section of this course. If you do not focus on a previous patient case study, please contact the course faculty for patient approval.

Develop a PowerPoint presentation that addresses each of the elements listed below.

  1. Introduction (23 slides)
  • Introduce yourself to the audience.
  • Introduce the concept of health maintenance and its importance in promoting wellness and disease prevention.
  • Provide an overview of the purpose and goals of the health maintenance plan
  • List at least three goals for the presentation using Blooms Taxonomy verbs.
  1. Patient Assessment (23 slides)
  • Introduce your patient and describe the case scenario.
  • Complete a comprehensive patient assessment, including demographic information, medical history, family history, and lifestyle factors.
  • Identify three specific health risks and potential areas for intervention based on the patient assessment.
  1. Health Maintenance Plan Development (34 slides)
  • Develop a comprehensive health maintenance plan that addresses the identified health risks and areas for intervention.
  • Incorporate evidence-based strategies for disease prevention and health promotion, including recommended screenings, immunizations, and lifestyle modifications.
  • Discuss the importance of patient-centered care, cultural competence, and health literacy in the development of the health maintenance plan.

  • Develop a detailed plan for implementing the health maintenance plan, including timelines, resource requirements, and staff roles and responsibilities.
